is this a real secret in the game? how do you do it?

The secret area is real, you don't need cheats to get to it (walk off an elevator as its going up), and those are pictures of the dev team, not kidnapped and tortured people.

the 'blood splat' on the screen isn't real, the music stopping isn't real, the sound disappearing isn't real...at least not pertaining to the secret.

Since Charles Jacobi (Chukker) is a member of these forums, maybe you should ask him if he was kidnapped and tortured. 

You don't need hacks to get to the room either.  There is a misaligned wall texture visible on top of a large crate on E1M2.  You can get to the top of the crate by running off the lift next to it when it is either going up or down.  You then open the door behind the misaligned texture and follow the duct into the room.
